We provide highly qualified Registered Nurses and Licensed Vocational Nurses who offer skilled care and essential instructions. This enables our patients to function independently while increasing their comfort and overall quality of life.  Also, the patient's primary physician is kept in close communication to ensure the delivery of health care.

Physical, Occupational, & Speech Therapy

 

Physical, Occupational, and Speech Therapists are specially trained to assist our patients with improving their abilities to perform daily activities. Our therapists will assist family members in making arrangements for out-patient services while assessing the needs of the patients home environment.

Certified Home Health Aide

 

Certified Home Health Aides assist our patients with activities such as daily living, range of motion, strength exercises, personal care, meal preparation, walking, bathing, toileting, and dressing.​

Medical Social Service

 

Social workers provide counseling for patients and their family in dealing with the social and emotional aspects related to each patients' specific illness. Our specialists help family members to properly care for the patient by communicating the available community resource along with coping skills assessment.  â€‹

Community Support Services (CS)

​

CS: Respite Care: Providing Family and Primary Caregiver relief to avoid caregiver burnout. Our Respite Care Team has trained and dedicated Home Health Aides to provide compassionate care at the patient's home. 

​

CS: Home Maker/Personal Care/IHSS: Our trained caregivers are available to assist you in different areas of your activities of daily living while enjoying the comfort of your own home.

​

CS: Day Habilitation: Our well-designed practical life skills program is for those who recently suffered from debilitating condition. Our Occupational Therapist trained community health workers conducts the actual practice with patients to safely and effectively integrate back to the community.
